SUMMER BREAK Work Permit ANNOUNCEMENT - YOUTH SOURCE CENTERS
The Work Experience Education Office AND some YOUTH SOURCE offices will be open during Summer Break to support students seeking a work permit.

** Be sure to call or email the contact person to make an appointment. They will tell you what to bring to the appointment.
The Work Experience Education Office or YOUTH CENTER will issue a TEMPORARY WORK PERMIT valid through August 18, 2023.
DOCUMENTS NEEDED
- Completed Statement of Intent to Employ a Minor and Request for a Work Permit - Certificate of Age B1-1 Rev 02-14 (Form B1-1, Dated 02-14). One is provided below. Original Employer Signature and Parent Signature (and dates signed). No photocopy or digital signatures are allowed.
- Current Photo ID: Current Government ID or Passport, or Photo School ID. If the birthdate is not printed on the School ID, the student must also present the Certificate of Birth. The minor must have a physical copy of the ID. The Work Experience Education Office may not accept an ID from a device nor may the office accept an email copy.
VERY IMPORTANT
Be sure to visit Mr. Huezo when we return from SUMMER Break to RENEW the TEMPORARY Work Permit. You can come during 7th period with a pass or After School to Room 117.
WORK EXPERIENCE OFFICE
1. Please Download and Print the Work Permit Summer Application and take it to the employer to get their information and ORIGINAL SIGNATURE. A digital signature will not be accepted.
2. Your Parent needs to sign and date as well. A digital signature will not be accepted.
3. The Work Experience Office is located in downtown Los Angeles at 333 S. Beaudry Ave, Los Angeles CA 90017 (Office entrance is on 4th Street and Boylston Street).
