School Self-Review Checklist: Students with Disabilities

 
Review the five certification areas below which align to the 71 items from Attachment A (REF-2624). Then, certify each section on the adjacent column accordingly.
 

Section 1: Special Education School Teams & Designees

The school has developed a system to ensure compliance with special education legal requirements and has appointed the following designees/coordinators: 1) Special Education Designee; 2) SSPT Coordinator; 3) UCP Designee; 4) Section 504 Designee and 5) Nonpublic School Designee.
 

Section 2: Beginning of School Year Procedures

The school has informed staff of their responsibility to adhere to all instructional and service delivery requirements. The school informs the appropriate staff of the translation procedures for IEP team meetings and documents. The school ensures students with disabilities are afforded a FAPE and monitors the provision of all services by monitoring data using Welligent Management reports.
 

Section 3: Compliance Procedures

The school implements systems to meet the legal requirements of all IEPs. This includes but not limited to an established process understood by all staff members for referring students who may require special services, training for appropriate staff on the development of high quality IEPs, and the procedures for managing student records. Additionally, the school ensures parents are informed of their right to pursue various types of dispute resolution procedures and that there is no retaliation against parents or students who exercise their rights.
 

Section 4: Compliant Timelines

Assessment Plans are developed and provided within 15 calendar days of receipt of a request for a special education assessment. The school prioritizes the scheduling of IEPs by ensuring that all initial evaluations are conducted within 60 days. Additionally, as a best practice, the school schedules all annual IEPs at least 60 days before the due date and Triennial IEPs at least 90 days before the due date.
 

Section 5: Special Education Supports & Resources

The school uses the District's the Division of Special Education website and training materials/courses available on MyPLN as resources/supports to ensure all IEP-related items are of high quality and are implemented according to the specifications of the IEP. Additionally, the school uses the District's electronic Policies and Procedures (ePPM) Manual available in the Division of Special Education website. The school also uses eLibrary to retrieve information on mandates and policy documents.
